Privacy notice

Draft for clinic review. The clinic must approve and complete this notice before accepting real bookings.

Your information

Appointment forms collect your name, date of birth, contact information and selected appointment so the clinic can arrange your care. A verification code is sent only to your phone. Email is collected for account identification; it is not verified by email code. Optional text reminder consent is separate from booking and from marketing.

Patient access

Prescription access requires a verified patient account. Health information should only be available to you and authorized care personnel. Do not share your account or use a public device without signing out.

Optional measurement

This implementation does not load Google or Meta pixels. Optional aggregate marketing measurement is disabled by default. Patient forms, portal records and booking details are excluded from marketing events.

Maps and imagery

The map loads only when you select Load map. Google then receives connection information such as your IP address. No appointment details are shared with the map. Lifestyle images are illustrative models, not patient testimonials.

Questions and access requests

The clinic must add its privacy officer’s contact details here before launch.

Before launch

The clinic must specify its legal entity, custodianship, processors, data locations, retention schedule, disclosure practices, access and correction process, and complaint procedure in the approved notice.
